Pennsylvania has recorded 1,004 measles cases this year, marking the state’s largest outbreak in more than three decades, according to state health officials. Five residents have died after contracting the disease and 198 have been hospitalized.

Lancaster County is the hardest-hit area, with 391 confirmed cases. State officials said the outbreak began in April west of Philadelphia and attributed its spread to low vaccination rates. Gov. Josh Shapiro has expanded access to the measles, mumps and rubella vaccine in affected communities.

CBS News also reported the first confirmed measles infection in an Allegheny County resident.

The Pennsylvania figures are not included in the Centers for Disease Control and Prevention’s national count amid a dispute over the classification of measles-related deaths. The CDC reported 3,887 confirmed U.S. cases as of Oct. 1, making 2026 the country’s worst year for measles since 1991.

Neighboring states are also reporting outbreaks. New York Gov. Kathy Hochul declared a state of emergency Monday, citing 108 cases as of Oct. 3, while Ohio has recorded more than 200 cases this year. Health officials say vaccination coverage has fallen below the 95% level generally needed to prevent outbreaks.
