Personalised mRNA vaccine reduced melanoma recurrence risk in phase 3 trial

A personalised mRNA cancer vaccine given with pembrolizumab reduced the risk of recurrence or death among patients with high-risk melanoma by 42% compared with pembrolizumab alone, according to results from a large phase 3 trial.

The INTerpath-001 trial enrolled 1,137 patients with stage IIB to IV melanoma whose tumours had been surgically removed but who remained at high risk of the cancer returning. Participants had not previously received drug treatment for their cancer.

The vaccine, called intismeran, is individually produced using the unique genetic features of each patient’s tumour. It is designed to train the immune system to recognise and attack cancer cells. Unlike conventional vaccines that prevent disease, it is a therapeutic vaccine administered after a cancer diagnosis.

Researchers compared intismeran combined with the immunotherapy pembrolizumab against pembrolizumab alone. The findings are due to be presented by Professor Georgina Long of Melanoma Institute Australia and the University of Sydney at the European Society for Medical Oncology Congress 2026.

Long described the results as a new personalised approach to reducing the risk that melanoma will recur or lead to death. The BBC, New York Times, Washington Post and Yahoo also reported that the vaccine substantially lowered the likelihood of melanoma returning.
